It is Dr Dykema’s prescription policy that all of her patients be evaluated at least yearly. Some prescriptions or problems require more frequent evaluation. If you are patient and are due for an evaluation, please call our office to schedule an appointment.

Prescription refills that are approved by Dr Dykema will be called in to your pharmacy within 24 business hours. Prescriptions will not be filled after hours, on weekends, during holidays or office vacation.

If you are a patient or a pharmacy and requesting a prescription refill, please provide us with the following information.
  • Your name or the Patient’s name
  • The Patient’s Date of Birth
  • A phone number(s) where we can reach you if we have questions
  • Spell the prescription(s) requested
  • Include the strength, directions and amount
  • The name and location of the Pharmacy
  • The phone number of the pharmacy

    You may contact your pharmacy directly to check on the status of your prescription after business 24 hours.
