New Patients Dr Dykema is a family practice physician and has been in practice in northwest Phoenix for over 15 years. She is the only doctor in this office. She treats all ages of patients from newborn to adults. She evaluates and treats most illnesses and injuries and performs minor surgeries. Dr Dykema encourages all of her patients to adopt healthy lifestyles to reduce illnesses and prevent diseases. She does not prescribe diet drugs and amphetamines, narcotics or tranquilizers. Special exams performed include… Well woman physicals including pap exams Well man physicals including prostate exams Sports and school physicals DOT and FAA certification exams Immunization updates for children and adults Foreign Travel consultations Minor office surgeries Patients are seen by appointment and during our regular office hours only. The average wait for a new patient appointment is 2-7 days. Once you have been seen initially by Dr Dykema, we are routinely able to provide the same day or next day appointments for our sick patients. It is not necessary to have medical records from a previous doctor at the time of your initial appointment with Dr Dykema. However, you may request those records in advance and have them sent directly to you. We do not accept or review medical records from patients not yet seen. Dr Dykema manages patients in the office only and does not manage her patients when they are hospitalized. Hospital care of her patients is managed by specialists. The hospital specialists communicate with Dr Dykema by phone and fax to insure optimal hospital and post hospital care. So that we can verify your coverage prior to your visit Your copay or deductible amount in cash, credit card or check. It is our policy to collect in full prior to every visit A list of medications you are taking A list of medical problems if there is more than one We request that you leave children without scheduled appointments at home
